Equipment slides and fatally buries worker

An underground mine employee passed away after machinery slipped and became covered in a chemical mixture.

An excavator recently slid over an elevated loading pad and was engulfed in lime dust. The incident trapped a 50-year-old operator who died at the scene on 20 June 2022.

An official investigation suggested management could have better trained workers in identifying and reporting hazards.

“Conduct workplace examinations prior to beginning work and assure hazards are corrected,” the Department of Labor said in a safety alert.

“Stay clear of potentially unstable areas.”
