Authorities are examining circumstances surrounding an equipment-related workplace death.
Police, WorkSafe Western Australia and the State Department of Fire and Emergency Services recently investigated an employee fatality at a refinery in North Coogee – 27km southwest of downtown Perth.
They are trying to determine exactly how the WA Salt worker became caught on a 5 metre-long outdoor conveyer belt, behind a timber shed. The male is widely speculated to have died at the scene because no ambulance was seen transporting him to hospital on June 16.
“WorkSafe commissioner Darren Kavanagh said any work-related death was a tragedy, and relayed his sincere condolences to the man’s family,” the regulator said in a public statement.
“He was reported to have been working with a conveyor belt when he was caught in the belt. WorkSafe investigators [will] examine the circumstances of incidents with a view to ensuring compliance and preventing future incidents of a similar nature.”
Local media speculated the incident could have occurred near the bottom of the conveyor belt, because investigators had gathered around that area. Others were spotted using ladders to gather evidence.
